Documents Needed For Cashless Procecdure :- |

- Documents Related to Patient’s Medical Treatment.
- First Consultation Letter of Consultant Doctor.
- Latest Chest X-Ray Report / E.C.G. / Echo / T.M.T. Report .
- If Patient was transferred from any other Hospital , then last treatment papers with complete past medical history of the patient.
- If Patient was discharged from other hospital, then Discharge Card with complete details of diagnosis and related past history.
- Complete Medical File of Patient ( if any treatment taken for the same in the past)
Special Note :
- If Policy is Renewed within the period of One or Two Months before the Hospitalization Period, 64VB compliances Certificate / photocopy of Bank Passbook ( if premium paid by cheque ) / photocopy of Cash Receipt of Premium Payment from Insurance Company.
- If the patient is employee of any Corporate and the covering under Corporate group Policy, must require the details of current year policy, Employee ID No. On letterhead of Corporate.
Important Points :
- After Fulfilling the requirement of above-mentioned documents the process is going to be started. The Department assists to you to get the Authorization Letter for Guaranteed payment to provide Cashless Treatment for which we charge Rs.350/- (Rupees Three Hundred & Fifty Rupees only)
- The Cashless Request Form must Signed by the Patient / Patient's Relatives.
- On the day of Procedure /surgery , if your Cashless Request sent by us is in process by TPA or pending due to any reason , you are requested to DEPOSIT the Security cheque for the Package Amount at Billing Counter before Procedure /Surgery will start.
- If Any Query Raised by TPA , Related to Insurance Policy Details or any other Past Medical Treatment Related Documents , you must have to provide us at the earliest within 72 hours of the Query Letter received . Delay in submission of documents will be resulted in to Denial of Cashless Request by TPA for which Hospital is not rest responsible.
- Kindly provide us the Patient's Real Medical History, do not try to hide the same, If after investigation by us or by TPA , and we found that " NON DISCLOSURE OF MATERIAL FACTS " , the Cashless Denial and the Hospital Authority is not Rest Responsible for the same.
